School Nurse & Wellness Center


On Wednesday, July 8 at 2 pm, Dr. Schweitzer and I will be doing free sports physicals in the DHS Nurses Office for high school athletes only.   Due to the current pandemic and policies that must be followed, please read below and let me know if there are any questions.


 Students MUST sign up ahead of time for a time slot that day through my email (  It will be first come, first served basis.  If they do not sign up, they cannot participate.  They MUST wear a face mask and maintain social distancing policies while in the school.  They must come alone, no groups or extra people.  Anyone NOT getting a physical will have to wait outside. 


 Please have students check to see if/when their physical actually expires.  I can look that up quickly on Powerschool.  If they do NOT expire before November, they can wait for the next physical clinic (usually in November) to get theirs done.  If you have a roster that you want me to check, please forward it to me asap so I can update it. 


 Have your athletes email me if:  they need to check physical date OR to schedule a time for July 8.

Please note that the nurse DOES NOT carry allergy medicine or any form of cold medicine. If your child needs daily allergy medicine or has symptoms of a cold, those medications must be taken at home (unless a prior arrangement has been made with the nurse). Please remind your child to take their allergy medicine EVERY DAY DURING ALLERGY SEASONS.

ANY MEDICATIONS THAT NEEDS TO BE STORED AT OR TAKEN AT SCHOOL MUST ALSO HAVE A SIGNED MD ORDER FORM. That form can be downloaded from this website or you can call or email my office.


INHALERS: if your child has asthma and needs to use an inhaler at school, signed permission from the MD and a parent must be on file with the nurse. DHS Students can carry their own inhaler IF allowed, in writing, by MD and parent. Please contact the nurse with questions or for forms. 

EPIPENS: if your child requires an Epipen for anaphylactic allergies, signed permission from the MD as well as parent, authorizing use, must be on file with the nurse. Students are allowed to carry their Epipen if they have signed authorization to do so, documenting that they have been trained on its proper use. This must be signed by parent and MD.


 **Attention** All students entering their Junior year in the fall of 2020 are required by the State of CT and the Derby BOE to have on file with the nurse a current physical WITH UPDATED PROOF OF IMMUNIZATION. This physical must be completed and signed by an MD and dated after January 1 2018; sports physicals are not acceptable for this. The physical must be on the State of CT blue form, available from the nurse, Student Affairs Office or download here

Attention all college-bound students: 

Your college will require proof of immunizations for you to attend. You can request a copy from Nurse Carloni and it is recommended you keep a copy at home for future reference.
